Haemorrhoids / Piles

Haemorrhoids (also known as piles) are swollen and inflamed veins in the rectum and anus that cause discomfort, bleeding and itching. They may be internal (inside the rectum) or external (under the skin around the anus).

Symptoms commonly include bleeding during bowel movements, itching or irritation in the anal region, pain or discomfort, swelling around the anus, and leakage of faeces. Advanced haemorrhoids may prolapse — protruding outside the anus — and require specialist treatment.

Fistula-in-ano

An anal fistula is an abnormal tunnel or tract that forms between the inside of the anal canal and the skin surrounding the anus. It typically develops as a result of an infection that forms an abscess near the anus.

Symptoms include persistent pain and swelling near the anus, discharge of pus or blood, skin irritation around the anus, and sometimes fever. Anal fistulas rarely heal on their own and require specialist surgical intervention.

Fissure-in-ano

An anal fissure is a small tear or crack in the lining of the anal canal (anoderm). It is one of the most common causes of severe anal pain, particularly during and after bowel movements.

Common symptoms include sharp, intense pain during bowel movements, bright red blood on tissue or in the toilet bowl, a visible crack in the skin around the anus, and a small lump or skin tag near the fissure. Chronic fissures may not heal without treatment.

Pilonidal Sinus

A pilonidal sinus (or pilonidal cyst) is a small hole or tunnel in the skin at the top of the buttocks, where they divide (the natal cleft). It often contains hair and skin debris, and can become infected — forming a painful abscess.

Symptoms include pain, swelling, redness, and discharge near the tailbone. Without treatment, recurrence is common. Traditional surgery often involves wide excision and large open wounds with lengthy recovery periods.

Anal / Genital Warts (Condylomata)

Anal and genital warts (condylomata acuminata) are caused by the Human Papilloma Virus (HPV). They can appear as single or multiple growths around the anus, genitals, or groin area, and vary in size from barely visible to large clusters.

Symptoms include itching, bleeding, or discharge, and in some cases they may be asymptomatic. Warts that do not respond to topical creams, or large and recurring clusters, are best treated with laser removal.

Anal Intraepithelial Neoplasia (AIN)

Anal Intraepithelial Neoplasia (AIN) refers to precancerous changes in the cells lining the anal canal. It is predominantly caused by HPV infection and is classified as low-grade (AIN 1) or high-grade (AIN 2/3) based on severity.

AIN is often asymptomatic and discovered during routine screening or investigation for other conditions. It is important to treat high-grade AIN to prevent progression to anal cancer.

Perianal Skin Tags

Perianal skin tags are small, soft flaps of skin that hang off the skin around the anus. They are benign (non-cancerous) and often develop following an episode of haemorrhoids, anal fissures, or anal surgery. While not dangerous, they can cause hygiene difficulties, irritation, or cosmetic concern.

Treatment is recommended when skin tags cause discomfort or ongoing symptoms. Dr Chamisa can assess and treat perianal skin tags with minimal intervention, discussing the most appropriate approach during a consultation.

Hidradenitis Suppurativa

Hidradenitis Suppurativa (HS) is a chronic, inflammatory skin condition characterised by painful, recurring lumps, abscesses, and tunnels (sinus tracts) under the skin. It typically affects areas with apocrine glands, including the axillae, groin, buttocks, and perianal region.

Traditional treatment often involves wide surgical excision, resulting in large wounds and prolonged recovery. Modern laser-based treatment offers a far less invasive alternative.

Dr Chamisa uses laser ablation to selectively destroy the primary fistula tracts in HS while sparing healthy surrounding tissue — avoiding extensive surgical excision and dramatically reducing recovery time. Optimal outcomes are often achieved in combination with biologics therapy to suppress the inflammatory stages of the disease.
